The on-site biometric health screening at Annual Conference will be Monday, June 13 and Tuesday, June 14 from 7:30 am –12:00 pm. Participants on the Conference active health insurance plan who have not registered for an appointment and would like to have your screening at Annual Conference, please visit https://www.chcw.com/ to access your CHC account and schedule an appointment. Scheduled appointments will have priority.
All others who are not participants in the conference health plan are welcome to complete a health screening on a walk-in basis. Payment is required prior to the event date. Please click here to pay. The cost is $76.00 per person.

The deadline for the Health & Lifestyle Assessment (located in the CHC account) and the Wellness biometric screening is August 31, 2022 to avoid a monthly $50.00 personal wellness surcharge beginning January 2023.
