Biometric Health Screening

Due to a change in providers, procedures regarding the biometric health screenings are slightly different this year. Please read the following information carefully. 


The biometric health screenings will be held on Monday, June 8, and Tuesday, June 9, of the 2020  Annual Conference from 7:00 a.m. – 2:00 p.m. Please note that this is a FASTING health screening. 

For clergy, laity, and dependents on the Conference health plan, there is no fee involved. Screening cost for those not on the Conference insurance plan is $91 for men; $76 for women. Please pay at the health screening check-in table, not main registration. Checks can be made payable to AWF Conference.

Clergy enrolled in the Conference active health insurance plan are required to fulfill two requirements: (1) an Online Health Assessment and (2) a Biometric Health Screening by August 31, 2020 to avoid a $50.00 monthly personal wellness surcharge. The health assessment should be completed prior to the health screening at Annual Conference. Clergy and lay staff on the Conference health insurance plan must schedule an appointment via the wellness website (https://app.chcw.com/#individual) for the biometric health screening on Monday, June 8, or Tuesday, June 9, from 7:00 a.m. until 2:00 p.m. Appointments may only be made through the CHC online registration system. Walk-ins will be serviced after scheduled appointments as time allows. The registration system will close for appointment making on June 6 at 12:00 p.m. 

**Please note that conference staff are unable to access individual records in the CHC system and therefore are unable to make, view, or alter appointment information.
