• Churches interested in becoming a sponsor for lay employee health/dental coverage should contact BeLinda Carnegie for eligibility requirements.
• Health insurance coverage is administrated through Blue Cross Blue Shield of Alabama, a third party administer of the conference self-insured plan.
• Dental coverage is administrated through Prudential Benefit Services.
The deadline for enrollment forms to be mailed or faxed is Friday, December 16th. The effective date of coverage will be January 1, 2012.
